• Birth Certificate
• Social Security Card
• TN Shot Record (you can get a copy from the Health Dept if needed)
• 2 Proofs of Residency (exa. include but are not limited to - mortgage statement, utility bills, rental agreement)
• Withdrawal form from previous school
